Appointments

We see patients by appointment. Please notify us at least 24 hours before your scheduled appointment time if you should need to cancel so that we may accommodate someone else in need.


Patient Forms & REGISTRATION

Upon scheduling an appointment, patients will receive an email invitation to the patient portal with access to patient paperwork. Please arrive 15 minutes early to your appointment with the following items with you:

  • Government Issued ID
  • Insurance Card
  • List of any prescription or over-the-counter medications you are currently taking

Financial Policy

All co-payments, deductibles and non-covered services are required to be paid at the time of service. For your convenience, we do accept checks, cash, debit and credit cards.


Prescription Refills

When requesting refills, please be prepared to provide us with:

  • the name of the medication and dosage
  • phone number for your pharmacy

Please allow 24 hours for your prescription refill to be reviewed and phoned in. As a general rule, we do not process refill requests after hours or on the weekends unless it is an emergency.


Insurance

We accept most major health insurance plans, please contact your insurance provider to confirm we are part of your plan.

AAASF accreditation

Guthrie Facial Plastic Surgery is proud to say that we have our own accredited operating suite in our office. This provides not only a private, comfortable setting for our patients to undergo surgery, but also an exceedingly safe environment. We took an extra measure beyond the required Tennessee state licensing, and demonstrated our commitment to patient safety by gaining accreditation from the American Association for Accreditation of Ambulatory Surgery Facilities (AAAASF).

AAAASF is dedicated to promoting the highest quality patient safety in the ambulatory surgery setting (office-based or outpatient).

AAAASF requires 100% compliance with all accreditation standards and does not offer partial accreditation. If a facility fails to meet any standard, it must correct the deficiency or risk denial or loss of accreditation.

To earn its accreditation, Guthrie Facial Plastic Surgery was inspected, and passed, on hundreds of critical criteria concerning patient safety. AAAASF facility inspectors are board-certified medical specialists trained to perform a thorough and complete facility inspection.

AAAASF facility inspectors follow a regimented procedure to inspect an outpatient surgery facility in the following categories:

General Environment
Operating Room Environment, Policy and Procedures
Recovery Room Environment, Policy and Procedures
General Safety in the Facility
Blood and Medications
Medical Records
Quality Assessment and Quality Improvement
Personnel
Anesthesia


AAAASF was established in 1980 to standardize and improve the quality of medical and surgical care in outpatient facilities and assure the public that patient safety is top priority in an accredited facility.

More than 2,300 outpatient facilities are accredited by AAAASF, one of the largest not-for-profit accrediting organizations in the United States. Surgeons, legislators, state and national health agencies, and patients acknowledge that AAAASF sets the "gold standard" for quality patient care.
